What counts as leaving

The chief financial officer or principal financial officer, or the chief executive officer or principal executive officer, resigns, retires, is removed or stops serving in that role. Interim holders count. So does a move to another role at the same company.

Reason, date, successor

Given where both readings agree. The date is the one the filing gives for the person leaving the role.
